On the first visit, all clients will be asked to fill out an intake form. None of your information will ever be shared. This form is absolutely necessary to ensure that you receive the maximum benefit from your massage session and that you have no conditions for which massage would be inadvisable. Refusal to fill out this form will result in termination of the session.

The massage offered by Common Source Massage Therapy is strictly therapeutic and completely non-sexual. Any inappropriate comments or sexual advances will not be tolerated and will result in termination of the session with full payment.

24 hours notice is required for cancellation of an appointment. If less than 24 hours notice is given, full payment will be due. There is no penalty for rescheduling an appointment.
